The deadline for retired members to submit an address change to CTPF for the 2023 1099-R mailing is December 15, 2023. Address changes cannot be made over the phone.

Retirees can make address changes in the myCTPF Member Self-Service Portal or complete a CTPF Change of Address form. Call Member Services at 312-641-4464 to request a form. Your identity will be verified before the form is issued. 

Completed forms can be returned by email to imaging@ctpf.org, fax, or mail.
