2024 Health Insurance Rebate Reminder

Members who received and paid for health insurance coverage from another source between January 1-December 31, 2024, recently received a mailing detailing the process of applying for a health insurance premium subsidy. Applicants must complete and return a Rebate Application (Form 355) with supporting documentation. Please note: If you were enrolled in a group plan, you must also submit a Group Health Insurance Premium Verification (Form 354). You must have Form 355 on file or accompanying Form 354 for it to be accepted at CTPF. The deadline to submit applications is July 31, 2025 (with NO exceptions). Learn more at ctpf.org/ctpfsubsidy. 

Health Insurance Rebate Webinars 

CTPF will offer webinars that will walk members through the process of applying for an outside rebate. These webinars provide a general overview of the eligibility requirements, required documentation, the timeline for the rebate process, and pitfalls to avoid when completing the outside rebate application. Webinars will be held on March 19, 2025, at 1:30 p.m. and March 26, 2025, at 10:00 a.m. and will include time for Q&A. Visit ctpf.org/calendar to register.
