Health Insurance 2025 Rates

The Health Insurance Premium Subsidy is set annually by the Board of Trustees. The subsidy will remain at 60% in 2025. The subsidy is only available to eligible CTPF members. Dependent coverage is not eligible for the subsidy.  

Medicare Plans 2025 Rates (member rate)   

The following rates are effective January 1, 2025. They represent the cost of single coverage and include the 60% premium subsidy: 

UnitedHealthcare Group Medicare Advantage PPO with Express Scripts Medicare® (PDP) $92.59 (was $72.17) 

Humana Group Medicare HMO with Part D Pharmacy $57.56 (was $52.34) 

Cigna Surround Group Supplement Plan with Express Scripts Medicare® $161.62 (was $135.90)  

AARP Medicare Supplement Plan F* (UnitedHealthcare) with Express Scripts  

Medicare® (PDP), average by age:   

  • Age 69-71  $190.92 (was $170.50) 
  • Age 72-74  $208.03  (was $187.61) 
  • Age 75+  $222.88 (was $202.46) 

*Premiums for the AARP Medicare Supplement Plan F are based on age, gender, vary by geographic area, and are quoted directly by UnitedHealthcare. The amounts listed above are average costs and include the subsidy and prescription drug premium. Additional plan increases may occur in June of 2025 and notice will be sent directly from UHC in the spring, and the prices quoted from UHC do not include the 60% CTPF health insurance premium subsidy or prescription drug premium. 

Medicare Premiums 

The Centers for Medicare and Medicaid Services (CMS) announced Medicare premium rates for 2025. The standard monthly premium for Medicare Part B enrollees will be $185.00, an increase of $10.30 from $174.70 in 2024. The annual deductible for all Medicare Part B beneficiaries will be $257 in 2025, an increase of $17 from the annual deductible of $240 in 2024.  

All CTPF members receive the same Part B subsidy. CTPF will provide a subsidy of $109.20 per month, in 2025, for all qualified members. If you make Part B payments directly to Medicare, CTPF provides the subsidy by adding $109.20 to your monthly pension benefit. Some members are subject to IRMAA fees. CTPF does not subsidize any Medicare fees, Part D Late Enrollment Penalties (LEP) or Part B or Part D IRMAA amounts. 

Non-Medicare Plans 2025 Rates (member rate)   

CTPF is offering a single Non-Medicare BCBS PPO plan (with some plan changes), and eliminated the UHC Non-Medicare PPO. The BCBS HMO is unchanged and will also be offered. Members enrolled in a CTPF Non-Medicare PPO plan, were automatically enrolled in the new BCBS Non-Medicare PPO effective January 1, 2025, unless they elected to move to the HMO plan or disenroll.  

Blue Cross and Blue Shield PPO $661.65 (was $1,016.36) 

Blue Cross and Blue Shield HMO Illinois $449.45 (was $438.06) 

2024 Health Insurance Rebates 

Members who received and paid for health insurance coverage from another source between January 1 - December 31, 2024, will receive a mailing this spring detailing the process of applying for a health insurance premium subsidy. Applicants must complete and return Form 355. The deadline to submit applications is July 31, 2025, (with NO exceptions). 

HIPAA Authorized Representative Form 

If you want a family member to assist you with health insurance issues, you can submit a HIPAA Authorized Representative Designation form (Form 345), available at ctpf.org or from Member Services. Your Authorized Representative may discuss your health insurance options, but cannot make care or treatment decisions.
