Health Insurance Premium Subsidy

Members enrolled in non-CTPF health insurance plans and/or Medicare may be eligible for a subsidy, subject to maximum reimbursement amounts published annually. The maximum reimbursement amount will be based on CTPF’s least expensive Medicare or Non-Medicare plan option. An application is mailed annually in the Spring to members not enrolled in a CTPF health insurance plan. The subsidy is paid out retroactively in an annual payment. Premium payment documentation is required and is explained on the application.  

How to Speed Up the Processing of Your Application 

Submit forms and documents by fax 312-641-7185 or email an attachment (.pdf format) to imaging@ctpf.org. Submitting forms electronically will ensure prompt processing. Members mark the information on their documentation that does not scan well and we cannot read the information. Refrain from marking over information. If you return an application without supporting documentation, your application will be rejected. If you do not resubmit a new application with supporting documentation before the July 31st deadline, you will not be considered for a rebate subsidy review and possible payment.  

Proof Made Easy

The Health Insurance Subsidy Preferred Proof List & Examples document shows the PREFERRED PROOF for members to submit. Please note that while other proof is acceptable, as outlined in the document, submitting the PREFERRED PROOF will reduce the turnaround time for your application to be processed. The form of proof must show the premium paid for the coverage period.   

CTPF reserves the right to request additional information, including proof of premium billed, if needed. 

In addition, CTPF does review prior year’s applications for consistency. If your previous year’s rebate proof was approved, CTPF recommends that you send in the same documentation for the current rebate year.   

Need A Group Plan Application (Form 354) 

If you were enrolled in a group plan, you must submit a Group Health Insurance Premium Verification (Form 354)

Reminder: You must have a Rebate Application (Form 355) on file or accompanying the Form 354 for it to be accepted at CTPF. If you only send a Form 354, your rebate application will be rejected without review or consideration for a possible payment.
